Important Office Policies:
1. Copayments are due at the window at each visit. We accept cash, credit cards, and debit cards. Our preference is debit Mastercard or Visa.
2. Updates to demographic information is required at each visit. Let our staff know if you have changed your address, telephone number, email, insurance, or any other important information.
3. You will be asked to present a copy of your current insurance card at each visit.
4. If you cancel or fail to show up for a scheduled appointment, you will be charged a $50 cancellation fee. No exceptions. This fee will be waived if you contact our office at least 24 hours prior to your scheduled appointment.
5. Office hours are Monday through Friday from 9:00 AM until 5:00 PM. Dr. Aguilar is offering limited Saturday appointments with a service charge of $15 (added to any regular fees). Please call the office to request your Saturday appointment as space is limited.
6. During Flu season (late September through December) you may schedule a Saturday appointment for your flu shot without the additional Saturday service fee of $15.
