NEW PATIENTS:
If you are a new patient, please arrive at least 15 minutes early to complete the required paperwork for your visit. You will be asked to show your current drivers license and a copy of your current insurance card.
All new patients are required to print and complete the following forms prior to your scheduled visit. If you have any questions regarding these forms please call our office. You may fax these forms to our office anytime prior to your scheduled visit to minimize your wait time.
RETURNING PATIENTS:
If you are a returning patient, please be aware that we have updated our computer and billing system. We will need to verify the information currently on file for you so please arrive at least 15 minutes prior to your scheduled appointment.
If you are a returning patient and have changes to your name, address or insurance information, please print and complete the forms below prior to your scheduled visit as well. You may fax these forms to our office anytime prior to your scheduled visit or you may bring them with you 15 minutes prior to your scheduled appointment.
