If you were a patient of Dr. Aguilar's and would like a copy of
your medical records,  please submit (via fax) a medical records request form and
fax it to the following fax number: (714) 389-3638.  The medical request form is listed below in the Documents Library.

Dr. Aguilar's staff will contact you with the fees associated with copying your specific medical chart using the fee schedule set forth by the Medical Board of California.  If you have any questions, please fax a letter to the doctor at the fax number above and someone will get back to you as soon as possible.
